Article 2 Land Survey Certificate
Land Survey Certificate is a document recording information about a land plot that has undergone land survey and measurement of people within the village or state organizations, with agreement from the administrative authority or relevant sectors according to regulations.
Land Survey Certificate is the primary evidence document regarding land use rights, which has attached copies of documents certifying the recognition of land use rights within forest areas correctly, and handed over to the owner of the land use rights for use as evidence according to regulations.

Article 2 Printing of Land Survey and Measurement Certificate Forms
The District Agriculture and Environment Office is directly responsible for printing the land survey and measurement certificate forms.
The printing of land survey and measurement certificates shall comply with Article 4 of this agreement, using the Lao Land Hack land registration database program that can connect to the citizen and identity card database of the Ministry of Public Security.

Article 6 Printing of Details on the Land Survey and Measurement Certificate Form
The printing of details on the land survey and measurement certificate form shall be carried out in accordance with Article 4 of this agreement, using the land registration database program Lao Land Hack (LLT) as follows:
1. Number: Print according to the sequential number of issuance of the land survey and measurement certificate for each plot, as per the order within the village;
2. Plot location: Print the code numbers of the province, district, sub-district, and village where the land is located, consisting of nine digits, where the first two digits are the code of the province or Vientiane Capital, the middle three digits are the code of the district or city, and the last four digits are the code of the village where the land is located;
3. Issued to: Print the name of the party-state organization, individual, legal entity, or partnership;
4. Acquisition of forest land use right: Print according to the name of the agreement granting the forest land use right, including the number and date of such agreement;
5. Type of forest land: Print the name according to each type of forest land, such as: national protection forest, provincial, district, sub-district, and village protection forest; national conservation forest, provincial, district, sub-district, and village conservation forest; and national production forest. For the zone, print according to the location of the plot, such as plain, plateau, and mountainous area, and clearly define such zone, e.g., urban, suburban, and rural;
6. Objective of forest land use: Print according to the objective of granting the forest land use right, such as: construction land, paddy field land, agricultural crop plantation land, livestock pen land, tree plantation land, fish pond land;
7. Cadastral map number: Print information according to form "Kor 2" using the land registration database program Lao Land Hack (LLT);
8. Plot number, land area, scale: Use the land registration database program Lao Land Hack (LLT) as follows:
8.1 Plot number: Print information according to form "Kor 2";
8.2 Land area: Print information according to form "Kor 2", with the unit printed as square meters;
8.3 Scale: Print information according to form "Kor 2".
9. Issued at: Print the name of the district/city where the land is located;
10. Date: Print according to the date of issuance of the land survey and measurement certificate after the signature of the head of the District Agriculture and Environment Office. In case the head of the District Agriculture and Environment Office has delegated authority to the deputy responsible for the work temporarily, before signing the land survey and measurement certificate, write the words "Sign on behalf of" above the position of the office head, and write the words "Deputy Head of Office" below the office head.
